Health Forms: For immunization records you can ask the Dr. for a copy of the immunization documentation they keep on file. For all children you also need a General Health Appraisal signed by a health care provider. Here is a link where you can print the form in case you want to bring it with to your appointment and save yourself an additional trip to the Dr. http://www.healthychildcare.org/pdf/HealthForm.pdf.
